Youth Programs
The Evansville United Methodist Church has a fully-active youth program, including a youth group, grade-school (K5-8), Junior- and Senior-high Sunday School program. Sunday school runs from the first Sunday after Labor Day through the last Sunday before Memorial Day.
Youth Medical Release Form The Evansville United Methodist Church standard Medical Release form for all youth activities must be signed and in the church office prior to any youth activity. This must be updated annually. You can find the Medical Release form here.
Youth Permission Slip The Evansville United Methodist Church standard permission slip form must be signed for each activity the youth is attending. You may find the Permission Slip here.
